Puddleby-in-the-Marsh is the residence of Dr Dolittle and krew: viz Dab Dab the duck, Gub Gub the pig and so on.

Haw, I got impatient by this one and checked out Another Forum... and even then it took me minutes to get what the answers were about. Tiny hint: "going out" and "coming in" are indeed relevant, though not in the sense of births/deaths.

Since next set is up, I guess I can affirm that yes, this thing is about golf holes (nos 1-9 of their respective courses on the way out, 10-18 on the way in) named after stuff. Seems pretty damn impossible for anyone who isn't intimately familiar with the great courses, except our host got one anyway: Doolittle is apparently hole 12 (http://www.manorhousegolf.co.uk/EXCLUSIVE_HOTELS/the_club/the_course/hole_by_hole_guide.aspx) at the Manor House Golf Club in Wiltshire:
